As an MDS Nurse at JAG Healthcare, you’ll play a vital role in ensuring every resident receives individualized care while supporting accurate assessments, quality outcomes, and regulatory compliance.
Working closely with physicians, nursing staff, rehabilitation therapists, social services, dietary, and facility leadership, you’ll coordinate resident assessments, develop care plans, and ensure documentation accurately reflects each resident’s needs.
This position combines clinical expertise, attention to detail, collaboration, and critical thinking to help ensure residents receive the highest quality of care while supporting the success of the entire healthcare team.

Qualifications
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Ohio (depending on facility requirements)
- Previous experience in long-term care or skilled nursing preferred
- Previous MDS experience preferred
- Knowledge of CMS regulations, MDS assessments, and care planning preferred
- Strong clinical assessment and critical thinking skills
- Excellent organizational and time management abilities
- Strong written and verbal communication skills
- Detail-oriented with excellent documentation skills
- Ability to work independently while collaborating with an interdisciplinary team
