As a Social Service Designee at JAG Healthcare, you’ll serve as an advocate, resource, and source of support for residents and their families throughout their stay. Working alongside the interdisciplinary care team, you’ll help residents navigate transitions, access resources, address concerns, and maintain the highest possible quality of life.
Whether you’re helping a new resident adjust to their new home, coordinating discharge planning, or simply offering a listening ear, your compassion and communication skills help residents feel heard, respected, and supported every step of the way.

Qualifications
- High school diploma required; associate’s or bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred
- Previous experience in long-term care, healthcare, social services, or case management is preferred
- Excellent communication and interpersonal skills
- Strong organizational and documentation abilities
- Ability to work collaboratively with residents, families, and interdisciplinary team members
- Compassionate, patient, and professional demeanor
- Knowledge of resident rights, confidentiality, and applicable regulations is preferred
- Dependable and committed to providing exceptional resident support
